Canadian Tennis Hall of Fame - Player Nomination Form

In order to be nominated under the Player category, players are to be retired at least 3 years from competition and shall be chosen on the strength of their competitive records with consideration given to sportsmanship and character. Under exceptional circumstances, the HOF Committee and Board may waive the 3-year provision.
